Routine Fees: Fees are based on the number and complexity of your dermatological problems. You will be given a receipt for your private insurance so that you may be reimbursed.
We accept Medicare assignments for all eligible patients. You are responsible for any co-payment and deductible portions of insurance coverage. Since cosmetic procedures are not routinely covered by insurance companies, payment is due at completion of your visit. If you have previously been our patient, but have not been seen for several years, this may be billed as a new visit.
Surgical Fees: We will bill Medicare, Blue Shield, or your private insurance company for all procedures except those of a cosmetic nature. Please provide any necessary forms and addresses required by your insurance company. If there is any deductible or co-insurance amount, you will be billed.
If you do not provide your insurance information, or a HMO referral form if appropriate, you will be responsible for the entire balance. If your visit includes an evaluation for a medical problem other than your surgical problem, a charge for the office visit may be made in addition to the surgical fee.
Managed Care and HMO Insurance: We consider it your responsibility to be aware of the terms of your insurance contract and to obtain necessary referrals from your primary care physician. If your insurance plan requires a referral, please be sure you have a timely and valid referral at the time of each visit. We will be happy to assist you with this process.
Billing Office: Any questions concerning your bill or insurance balance are best answered by our billing department staff. Someone is available at (610) 565-2640 Monday through Friday from 8:00 a.m. to 3:00 p.m. Please keep us informed of any insurance or address changes.
